INTRODUCTION
 
This document contains the following Parts:
 
A.     Assumption of Risk and Release Form – Quebec Residents
B.     Assumption of Risk and Release Form – Non-Quebec Residents 
C.     Code of Conduct and Ethics Consent Form
D.     Image Consent Form
E.      Privacy Policy Consent Form
 
The relevant Parts listed above must be signed by Participants before participating in the business, activities, and Events of Rowing Canada Aviron (RCA) and its Members (as applicable).
 
Due to differences in the laws of Quebec and the rest of Canada, Participants must sign the Assumption of Risk and Release Form specific to the location of their residence. Accordingly, residents of Quebec are required to sign Parts A, C, D and E. Residents of all other Provinces and Territories are required to sign Parts B, C, D, and E.
 
Please read each Part carefully before signing.
 
DEFINITIONS
 
“Participants” means athletes, coaches, other Athlete Support Personnel, or any other persons directed by Rowing Canada Aviron ("RCA") to sign any Part of this document.  
 
Except where specifically defined herein, all other capitalized terms used in this document shall have the meanings set out in the RCA Safe Sport Policy Manual, avaliable on the RCA website.  
 
In signing the Parts of this document applicable to you or to the Minor Participant over whom you have parental and/or legal authority, you acknowledge that you have read and understand all Parts of this document applicable to you or to the Minor Participant over whom you have parental and/or legal authority. You further acknowledge that you have signed each applicable Part of this document voluntarily and in the knowledge that its terms are binding upon you, the Minor Participant, and your respective heirs, spouse, children, parents, guardians, next of kin, executors, administrators and legal or personal representatives (as applicable).  
 
In addition, you acknowledge that Participants are bound by and expected to company with all RCA policies, as they may be amended from time to time, as a condition of participating in the activities and Events of RCA and its Members (as applicable). This includes being bound by the Universal Code of Conduct to Prevent and Address Maltreatment in Sport (UCCMS) and, where so designated, subject to the Canadian Safe Sport Program (CSSP). Nothing in this document is intended to modify or limit this requirement to the policies specifically mentioned herein.  
 
This document may be signed electronically, in which case your electronic signature shall be as binding, valid and enforceable as an original.  
 

PART C | PARTIE C
CODE OF CONDUCT AND ETHICS CONSENT FORM
 
1. Rowing Canada Aviron ("RCA") has adopted a Code of Conduct and Ethics to ensure a safe and positive environment within the programs, activities, and Events of RCA and its Members by making Individuals aware that there is an expectation, at all times, of appropriate behaviour consistent with the applicable organization’s core values and policies.  
 
2. In signing below, you (or your parent or legal guardian, if you are a Minor) acknowledge and agree that:  
 
a. You have read and understand the Code of Conduct and Ethics, including, without limitation, its application and all responsibilities set out therein;  
b. You are responsible for knowing and complying with the  Universal Code of Conduct to Address Maltreatment in Sport (UCCMS), which is incorporated into the Code of Conduct and Ethics by reference as if set out in full therein;  
c. Where designated as a Canadian Safe Sport Program (“CSSP”) Participant, you are additionally responsible for knowing and complying with the  CSSP Rules;  
d. You are bound to all of the policies found in RCA’s Safe Sport Policy Manual that are not specifically referenced in this Part B and that you are able to access the full Policy Manual on RCA’s website; and  
e. Any Individual who violates the Code of Conduct and Ethics may be subject to sanctions pursuant to RCA’s Complaints and Discipline Policy. In addition to facing possible sanctions pursuant to the Complaints and Discipline Policy, an Individual who violates the Code of Conduct and Ethics during a competition may be removed from the competition or training area, and the Individual may be subject to further sanctions.  

PART D | PARTIE D
IMAGE CONSENT FORM
 
1. I (or my parent or legal guardian, if I am a Minor) hereby grant to Rowing Canada Aviron ("RCA") and, as applicable, its Members (collectively the “Organizations”) on a worldwide basis, the permission to photograph and/or record the Participant’s image and/or voice on still or motion picture film and/or audio tape (collectively the “Images”), and to use the Images to promote the sport and/or the Organizations through traditional media such as newsletters, websites, television, film, radio, print and/or display form, and through social media such as Instagram, Facebook, TikTok, YouTube, and X (formerly Twitter). I understand that I waive any claim to remuneration for use of audio/visual materials used for these purposes. This consent will remain in effect in perpetuity; however, consent may be withdrawn, by providing written notice as described below. 
 
2. I hereby fully release, discharge, and agree to save harmless the Organizations, for any and all claims, demands, actions, damages, losses or costs that might arise out of the collection, use or disclosure of the Images or taking, publication, distortion of the Images, negatives, and masters or any other likeness or representation of the Participant that may occur or be produced in the taking of said Images or in any subsequent processing thereof, including without limitation any claims for libel, passing off, misappropriation of personality or invasion of privacy. 
 
 
3. I UNDERSTAND AND AGREE,  
a. that I have read and understood the terms and conditions of this Part C. On behalf of me, my heirs, spouse, children, parents, guardians, next of kin, administrators and legal or personal representatives, I agree that I am signing this document voluntarily and will abide by such terms and conditions.  
b. that I may withdraw the consent expressed herein at any time by providing written notice to the RCA Privacy Officer. I acknowledge that any withdrawal of consent may affect RCA's ability to provide certain products or services to me. Further information on the implications of withdrawal can be obtained from the RCA Privacy Officer; however, without limiting the generality of the foregoing, I acknowledge that the Organizations may collect Images at activities and Events including, but not limited to, training and competitions, or may authorize third parties to do so, and that in such circumstances the Organizations cannot guarantee that the collection and/or subsequent use and disclosure of Images will not occur. Participation in such activities and Events will therefore be taken to imply consent; however, upon receipt of a written request, RCA will take reasonable steps to remove and/or direct that the Images no longer be used or disclosed. 

PART E | PARTIE E
PRIVACY POLICY CONSENT FORM 
 
1. In the course of providing programs and services, Rowing Canada Aviron ("RCA") may collect Personal Information, as defined in RCA’s Privacy Policy.  
 
2. By providing Personal Information to RCA, you (or your parent or legal guardian, if you are a Minor) agree and consent to the collection, use and disclosure your Personal Information in accordance with RCA's Privacy Policy.  
 
3. In addition to the disclosures of Personal Information contemplated by RCA’s Privacy Policy, Participants who are also Canadian Safe Sport Program ("CSSP") Participants (as defined in the RCA Safe Sport Policy Manual) acknowledge and agree that RCA may disclose Personal Information to Sport Integrity Canada, where required or requested by Sport Integrity Canada for the implementation of the CSSP.

ACKNOWLEDGEMENT
 
I FREELY ACCEPT AND FULLY ASSUME ALL STANDARD, COMMON AND REASONABLY FORESEEABLE RISKS, DANGERS AND HAZARDS INHERENT TO THE SPORT OF ROWING AND THE POSSIBILITY OF PERSONAL INJURY, ILLNESS, DEATH, OR PROPERTY DAMAGE OR LOSS RESULTING THEREFROM. I ACKNOWLEDGE THAT I AM RESPONSIBLE FOR MY CHOICE TO PARTICIPATE IN THE SPORT OF ROWING AFTER BEING NOTIFIED OF THE ABOVE-MENTIONED RISKS, DANGERS AND HAZARDS AND THAT THE LEVEL OF RISK, DANGER AND HAZARD THAT MAY AFFECT ME PERSONALLY MAY BE AFFECTED BY MY SKILL LEVEL AND EXPERIENCE IN THE SPORT OF ROWING, AS WELL AS ANY PRECAUTIONARY MEASURES THAT I MAY OR MAY NOT HAVE TAKEN TO AVOID SUCH RISKS, DANGERS AND HAZARDS AND MY CHOSEN REACTION IN ANY GIVEN SITUATION WHILE PARTICIPATING IN THE SPORT OF ROWING. 
FOR PARTICIPANTS WHO ARE MINORS (i.e. under 18 in Alberta, Manitoba, Ontario, PEI, Quebec, Saskatchewan and under 19 in other Provinces and Territories): 
As the parent or legal guardian of the Participant, I declare that I have parental and/or legal authority to sign this agreement on behalf of the Participant. I acknowledge that I have read and understand this agreement, and that I have been, by the present, made aware that I can seek legal advice regarding this agreement, that I have executed this agreement voluntarily, and that this agreement is to be binding upon the Participant, myself, and our respective heirs, spouse, children, parents, guardians, next of kin, executors, administrators and legal or personal representatives (as applicable). 
The undersigned, being the Participant and the Participant’s Parent/Guardian (collectively the “Parties”), acknowledge and agree to the terms outlined in this document.
